All US carriers are required to have filled driver application in files even if you are only driver in your own company. Below is download link from FMCSA or you can use management systems.
You may also use management system which makes easier to send applications to drivers. It also helps carriers stay on top of expire dates and ever changing requirements of data keeping. I prefer two of below:
